Before and after left breast revision in a 41-year-old woman with congenital breast asymmetry and past implant reconstructions. Her left implant had developed capsular contracture and asymmetry. She wanted to leave the right breast alone and only correct the left side.

Under local anesthesia, the previously placed left breast implant was removed and the implant space was reopened laterally via an internal capsulotomy incision. A new smooth round silicone gel breast implant was placed, and the left inframammary fold was reinforced using suture techniques to create improved symmetry.

Follow up photos are shown 1 year after surgery with improved symmetry to the breasts, and a happy patient - especially since she did not have to go to sleep for surgery!
