Before and after bilateral breast revision in a 49-year-old woman.  Her former mastopexy-augmentation had developed painful capsular contracture, asymmetry of the breasts, superior malposition where the implants sat up too high, and a Snoopy deformity/waterfall deformity of the breast tissue droops and falls off the unnaturally high implant on the chest.

Painful capsular contracture is classified as Grade IV out of IV on the Baker scale.  She had a physically demanding job and wanted relief of her pain, better ability to work without taking pain medication, and to keep implants – but to look more natural.

Bilateral capsulectomy removed constricting scar tissue from around the implants and created a new space for the implant to lower down in the breast.  New smooth round silicone gel breast implants were placed subglandularly, on top of the pectoralis major muscles.  Her old breast lift scars were redone to create better symmetry and remove excess baggy skin from the lower pole of her breasts.

Follow up photos are shown 6 months after surgery, with normal skin scars for this stage of healing.  They will gradually fade as the dilated blood vessels recede with additional scar maturation.  She is back working without any pain and feels comfortable wearing cute little tank tops without a bra!
