Before and after bilateral breast revision with liposuction of the axillary rolls in a 69-year-old woman. Her previously placed breast implants had developed capsular contracture with associated pain and deformity. The right implant sat up much higher than the left, which made her embarrassed to wear normal shirts that showed her upper chest.
She had developed bottoming out, with an abnormally high position of her nipple-areolar complexes on each breast. To complicate the situation, her left breast had been treated for breast cancer with a lumpectomy and radiation over a decade ago. This accentuated the asymmetry and made her left breast stiffer and more resistant to change.
Her goal was to keep implants and for her breasts to be a little fuller and lifted, with better symmetry. This revision involved bilateral capsulectomy and implant exchange from saline to smooth round silicone gel. A mastopexy lifted the position of her nipples and removed excess skin from the lower pole of her breasts. Liposuction removed excess fat from her armpits and bra rolls.
Follow up photos are shown 6 weeks after surgery, with fresh incisions and surgical swelling still evident – but greatly improved aesthetics and symmetry to her breasts. With time, her incisions will fade, and her body will resorb postoperative edema.
