Before and after bilateral breast augmentation and redo mastopexy with liposuction of the axillary rolls in a 55-year-old woman. She previously had a breast augmentation and lift in her thirties but decided to have her submuscular implants removed due to dissatisfaction with the cosmetic outcome.
Her original goal was to have more fullness in the upper pole of her breast, but the old implants sat way too high, had a wide space between them and moved into her armpits every time she flexed her chest muscles.
Now, years later, she sought a new surgeon to help her achieve her goals – fuller breasts with a natural look that were proportional to her curvy body. Instead of putting implants back under the muscle, a subglandular approach (our preferred technique) was used. Smooth round silicone gel breast implants were placed on top of the muscle through her existing scar under the breast to create improved upper pole fullness.
Her right nipple and areola was lifted to better match the height of the left nipple-areolar complex. Liposuction removed excess fat from her armpits and bra rolls through incisions hidden in skin creases. We always use drains when implants are inserted to help remove wound fluid from accumulating around the implant, which in turn decreases the risk of capsular contracture.
Follow up photos are shown 1 month after surgery. She did not have to wait for the implants to “drop and fluff” thanks to her drains, which were removed on postoperative day four. She feels like she’s “back in her body” and is very comfortable in her skin!
