Before and after bilateral breast reconstruction revision in a 56-year-old breast cancer survivor. She had undergone several past breast reconstruction procedures and was unhappy with the wide space between her breasts and the feel of her implants sitting on top of her ribs, in addition to muscle spasms due to the implants sitting under the pectoralis major muscle.
At the time of her mastectomies, her inframammary fold had been violated, causing her implants to slide down her chest and sit on her ribs, causing discomfort and a sensation that prevented her from exercising.
Her breast revision involved removal of her old implants, creation of a new prepectoral space, and reconstruction of her inframammary folds using suture techniques. A new smooth round silicone gel breast implant was placed on top of the muscle, and the lower pole and inframammary fold was reinforced with P4HB mesh.
This mesh holds its strength for around a year and a half, while the body grows scar tissue into this area. Once the mesh is completely gone, the lower pole tissue is strengthened, preventing bottoming out and recurrence of her implants slipping downward.
Liposuction removed excess fat from her bra rolls and armpits. Follow up photos are shown 6 months after surgery. Her animation deformity is gone, and she is much more comfortable. She loves the natural look of her breast reconstruction.
