Before and after bilateral explantation and lift in a 70-year-old woman with breast implants that had been in place for over twenty years and had developed capsular contracture, causing pain and deformity. She wanted to be smaller and lifted and look natural for a woman her age.

Bilateral explantation and total capsulectomy was performed under general anesthesia to remove the implants, extruded silicone due to rupture, and extremely thick scar tissue that made up the pocket. Her nipples were lifted, and excess skin was removed from the lower pole of the breast.

Liposuction removed excess fat from the bra rolls and armpits, a region known as the axillary rolls. Follow up photos are shown 6 months after surgery with fading scars and a natural, smaller breast volume and shape.
