Before and after bilateral nipple-sparing mastectomies and immediate breast implant reconstruction in a 39-year-old woman with breast cancer. She had completed neoadjuvant chemotherapy for an invasive tumor and was ready for surgical treatment of her cancer.

Her breasts had deflated completely after breastfeeding her three children, and she was excited to regain her former fullness back as part of her breast reconstruction. A breast lift approach was used for the mastectomy. Her breast tissue was removed through an incision under the breast.

At the same procedure, smooth round permanent and postoperatively adjustable breast implants were placed on top of the muscle, in the prepectoral position. They were fully inflated at the time of surgery to create a fuller, rounder breast reconstruction.

Since her breasts had significant droop, her nipples were preserved as free nipple-areolar grafts. They were reapplied at the end of the procedure as grafts and placed in a higher position on the breast mound. Her areolas were made slightly smaller as well.

Follow up photos are shown 6 months after surgery. She is very happy with her saline implants and declined the option to switch to silicone devices. She is considering having fat transfer (fat grafting) in a future procedure when she has her implant ports removed – but for now, she is satisfied with her breast reconstruction and is back focusing on being a Super Mom!
