Before and after bilateral nipple sparing mastectomies and immediate breast reconstruction in a 58 year old woman with the BRCA gene. Her aesthetic was natural and she did not want her breasts to look fake. She also had lost some weight and in turn, some breast volume. Her aesthetic goal was to be reconstructive slightly fuller than her preoperative breast size, similar to her breasts before menopause.

An inframammary fold incision was used for her nipple sparing mastectomies. Smooth round permanent and postoperatively adjustable implants were placed on top of the muscle, in the prepectoral position. A single inflation of saline was required postoperatively to achieve her final volume.

One year later, when she was ready to take time off work again for her port removal procedure, she elected to switch her saline filled breast implants to silicone for a more natural look and feel. Long term follow up photos are shown 10 years after surgery. She continues to come for follow up visits every 3-5 years now. She only requires another surgery if she is having problems – at age 68 years now, this is a relief to her!
