Before and after bilateral explantation and redo mastopexy in a 49-year-old woman.  Her implants were positioned wide on the chest and had a powerful animation deformity due to their submuscular placement and her powerful pectoralis major muscles.  These are two reasons why we do not like or recommend implants placed under the muscle, ever!

Other reasons we prefer prepectoral, subfascial or subglandular implants (which are essentially the same thing) include avoidance of implant malposition (abnormal positioning on the chest due to chronic muscle pulling), very little postoperative discomfort, no chance of other implant-related deformities such as a “double bubble”,  a natural upper pole slope and pleasing teardrop shape to the breast.

She had a very active lifestyle and no longer wanted her breast implants.  Breast implant removal was performed as a simple procedure under local anesthesia in our office operating room.  Her past breast lift scars were redone to achieve a modest breast lift at the same time.  She took a week off work and avoided lifting more than five pounds for two weeks.

Follow up photos are shown 3 months after surgery.  She resumed her insane workouts at three weeks postop and feels relief of her past muscle tightness – which she didn’t realize was happening until the implants were no longer there!
