Before and after bilateral explantation and conversion of breast reconstruction from implant-based to DIEP flaps in a 60 year old breast cancer survivor. She had undergone at least eight previous surgeries to treat capsular contracture. She wanted to be a little smaller than her current implant size; her original breasts before her mastectomies were small A cups.

Even though she was very thin and had an appendix scar on her abdominal wall, she had enough abdominal tissue to reconstruct two breasts! The old implants and scar tissue around the implants were removed, and DIEP flaps were microsurgically transplanted to the chest. After surgery, she got fit and lost some weight, but maintained a proportional breast reconstruction result.

Follow up photos are shown two years after surgery, with mature scars and no further implant-related complications!
