58-year-old woman with extensive left breast DCIS who wished to avoid radiation after a potential large lumpectomy. She chose to have a mastectomy and immediate reconstruction using her abdominal tissue. Her goal was to match her right breast size and shape as best as possible. A left nipple-sparing mastectomy and immediate DIEP flap reconstruction achieved excellent symmetry with her other breast. Postoperative images at one year are shown.
