44-year-old woman with a history of right-sided breast cancer, chemotherapy and radiation, and bilateral skin-sparing mastectomies with failed/inadequate implant reconstructions. She lost her right implant due to infection, and her left submuscular tissue expander was tight, painful and deformed. Conversion of an implant-based reconstruction to a bilateral DIEP flap reconstruction gave her symmetry, soft and natural-looking breast reconstructions using her abundant tummy skin and fat. Postoperative images after her nipples and areolas have been reconstructed are shown with scars that are still maturing.
