Before and after bilateral breast reduction and lift with liposuction of the axillary rolls in a 43-year-old woman. In the past, she had actually had two past breast reductions - one as a teenager and another in her early twenties. She felt that her surgeons never made her as small as she wanted to be, and she was looking for a woman Plastic Surgeon “who gets it” and actually listened to her goals! She had finished having all her children and was able to take a full month off for healing.

A redo breast reduction is called a “secondary reduction” (if it is the second time). In this case, it was a “tertiary breast reduction”. It was assumed that she had an “inferior pedicle” technique, based on the shape of the breast and the long scars. She never had liposuction previously with her surgeries, and was delighted that we add liposuction of the armpits and bra rolls to every breast reduction and lift surgery.

It is possible and safe to perform our standard “superomedial pedicle” technique (where the blood supply to the nipple and areola comes from either side of the sternum) in redo breast reductions where a different pedicle was used to supply in the past. It is important to preserve a robust blood supply to the nipple in all breast reductions cases, as we did for this patient, who thankfully did not have any postoperative healing issues.

Over a liter of fat was removed from her bra roll region and armpits, which complimented her breast reductions results. Follow up photos are shown a year and a half after surgery, with mature scars and her final results! She feels lighter, has no trouble jumping, running or exercising with just a light bralette on, and loves “her cute perky boobs”!
