Before and after bilateral breast augmentation and left mastopexy (breast lift) with liposuction of the axillary rolls in a 39-year-old woman. She experienced deflation of her breast volume on both sides after breastfeeding and weight loss and concurrent breast asymmetry. Her left breast sat lower than her right side and had an emptier, baggy lower pole. She wanted fuller breasts like she had before having children and better breast symmetry. She was also self-conscious about her right inverted nipple and wanted this corrected as well.

Smooth round silicone gel breast implants were placed on top of the muscle in the subglandular position, through an incision hidden under her breast - in the inframammary fold. A left skin-only inferior mastopexy “tucked” her lower pole skin to tighten it around the implant and achieve better symmetry with the right breast. This created a vertical scar extending downward from the bottom of her areola to the fold of the breast and resulting in a lift of the left nipple to better match the position of the right.

Liposuction removed excess fat from her armpits, and a tummy tuck with rectus diastasis repair performed at the same time with liposuction of surrounding areas completed her Mommy Makeover procedure. Correction of the right inverted nipple at the same time as putting in a breast implant was discouraged due to the potential increased risk of capsular contracture with stimulation of bacteria coming from the nipple.

Interestingly, the breast augmentation helped to pop out her right nipple and correct its inverted appearance. Follow up photos are shown 1 year after surgery after her augmentation-mastopexy, with fuller but natural-looking breasts and greatly improved breast and nipple symmetry.
