Before and after bilateral breast revision in a 60-year-old woman with breast asymmetry from a prior mastopexy and breast augmentation. Her right nipple and areola sat significantly lower than the left, and she had mild bottoming-out of both lower poles. She wanted to minimize surgery but have better symmetry so that when she wore her sports bras at the gym, her nipples would be at the same height.
Her revision involved redoing both inframammary fold scars in the folds of her breasts, lifting the right nipple and areola to better match the left, and redoing the vertical scar as well to tighten up the right lower pole of the breast. The last image shows the actual surgical markings.
This procedure was offered to her awake, under local anesthesia with an oral anti-anxiety medication and inhaled laughing gas. She was grateful that she did not need to fast before surgery (you can come right after a full meal), that she knew exactly what was done during surgery, and that she was not nauseated for a full day after a general anesthetic.
Follow up photos are shown 6 months after surgery, with greatly improved symmetry and a happy patient!
