Before and after bilateral breast reconstruction revision in a 52-year-old breast cancer survivor who had previously undergone a bilateral mastectomy and implant reconstruction. Her submuscular implants had a painful animation deformity which accentuated her breast asymmetry. Her breasts were flat and low; she was looking for relief of physical symptoms and a more aesthetic breast reconstruction result.
Her revision involved removal of her old implants, replacement of her pectoralis major muscles to the chest wall, and insertion of new smooth round silicone gel breast implants on top of the muscle, in the prepectoral position.
Capsulectomy removed excess scar tissue from around the implants. A breast lift was performed on both sides to create a rounder shape to each breast. This required creation of a new lollipop scar on the left breast and a vertical incision in the lower pole of the right breast to remove excess skin and tuck the lower pole.
Her right nipple was reconstructed using the “nipple sharing” technique, taking a piece of the right nipple and transplanting it to the left breast as a free nipple graft. The right areola was reconstructed via medical tattoo during surgery. Nowadays, we would refer this patient to one of our expert paramedical tattoo artists for an even nicer and more natural looking nipple and areola reconstruction!
Follow up photos are shown 4 months after surgery. She feels relieved of her pain and tightness and feels much more comfortable in her skin.
