Before and after bilateral breast implant reconstruction revision with liposuction of the axillary rolls in a 59-year-old breast cancer survivor. Her implants had a significant animation deformity due to their placement under the muscle. She wanted relief of her pain and tightness and sought us out due to our preference of always putting implants ON TOP of the chest muscle!
An attempt had been made to create a new nipple and areola after her non-skin-sparing mastectomies, but the tattoo had significantly faded, and the nipple reconstructions were positioned very low on the breast so that she could not see them! The surgical plan was to remove the old nipple reconstructions and excess skin as part of the surgery.
Her breast revision involved removal of her old implants, returning her pectoralis major muscles to the chest wall and creating a new prepectoral space for an implant on top of the muscle. New smooth round silicone gel breast implants were placed, recreating her original breast size and shape. Skin was removed from the lower pole of the breast, including the old tattoo circle. Liposuction removed excess fat from her armpits and bra rolls to complete the procedure.
Her nipples were reconstructed using the local flap technique. Medical tattoo created two new areola circles. Follow up photos are shown 1 year after surgery. If this surgery had been done today, we would have referred to a paramedical tattoo artist for the most natural and three-dimensional shading possible.
