Before and after bilateral breast revision with liposuction of the axillary rolls in a 41-year-old BRCA gene positive breast cancer “previvor”. She had previously undergone bilateral mastectomies with implant reconstruction in the submuscular position after surviving ovarian cancer and finding out she was BRCA+.
She experienced chronic pain and tightness in her chest due to her pectoralis major muscles constantly being pulled tightly over the implants. Her revision involved removal of the old overfilled saline breast implants, separation of her chest muscles from the undersurface of her mastectomy flaps, and reconstruction of her normal anatomy by suturing the pectoralis major muscles back onto their normal rib attachments.
New smooth round silicone gel breast implants were placed on top of the muscle, in the prepectoral position. Her nipples and areolas were lifted and improved symmetry of nipple height was created by redoing the lollipop scar from her past breast reduction. Liposuction removed excess fat from her armpits and bra rolls
Follow up photos are shown 3 months after surgery. She has begun scar therapy and is now able to use her arms normally for daily activities and has even taken up pickleball!
