Before and after bilateral breast revision in a 37-year-old breast cancer survivor. She had previously undergone bilateral nipple-sparing mastectomies, left breast radiation and chemotherapy. She suffered several postoperative infections, causing Grade IV capsular contracture that was associated with pain and deformity. Her left nipple was pointing toward the floor, which accentuated her asymmetry.
Her revision involved bilateral capsulectomy to remove internal scar tissue and reopen the implant space. New smooth round silicone gel breast implants were placed in the prepectoral position, on top of the muscle.
The left nipple and areola was moved up higher on the breast as a free graft – it was not possible to keep it attached to the breast as the distance it needed to be moved was too great. The lower pole of the left breast was tucked inward, creating a new vertical incision as for a breast lift, resulting in a new lollipop scar.
Follow up photos are shown 2 years after surgery, with very acceptable symmetry, especially given her history of radiation.
