Before and after bilateral breast reduction and auto-augmentation lift and liposuction of the axillary rolls in a 17-year-old woman. She felt her breasts were out of proportion to the rest of her body and made physical activities difficult and frustrating.
A breast reduction removed some excess lower pole breast tissue and fat but left a normal and proportional amount of tissue behind so that she maintained her body image. She and her parents were relieved to learn that a strong connection between the nipple and the rest of the breast is maintained, preserving the likely ability for future breastfeeding.
A local tissue rearrangement breast lift using the auto-augmentation technique was used to reshape her breasts and move the tissue up higher on her chest, as for a breast augmentation – instead, using our patient’s own tissue to fill the upper pole of the breast.
Liposuction removed fat from her armpits and bra rolls, allowing the scar in the fold of her breast to be as short as possible. Rather than surgically chasing her bra roll with a longer incision, we keep the inframammary fold incision short and debulk the fullness in the upper back with liposuction instead.
Follow up photos are shown 8 months after surgery, with normal immature pink scars and nipples and areolas that are still smooth and non-reactive. With time, the motor nerves to these structures will recover and her nipples and areolas will constrict naturally with cold and touch. Her scars will continue to fade and be less pink as the blood vessels recede during additional healing.
