Before and after bilateral breast implant revision with liposuction of the axillary rolls in a 36-year-old woman. Her old saline implants sat low on the chest, giving her a “matronly” look. She wanted more upper pole fullness and to be larger, with less “under-boob.”
Her revision involved removal of the old saline implants and insertion of slightly larger new smooth round silicone gel breast implants, keeping the same pocket. These added volume to the upper pole of her breast.
A small amount of excess lower pole skin and fat was removed at the same time, and her lollipop breast lift scars were redone to lift the nipples and areolas in a skin-only mastopexy. This combined augmentation and lower pole reduction is called the “plus-minus procedure,” where volume is added to the top of the breast but removed from the base to balance out the breast proportional.
Liposuction removed excess fat from her armpits and bra rolls. Follow up photos are shown 6 months after surgery, with normal immature pink incisions, generalized redness in the lower pole of her breast due to increased blood flow during healing and a fuller, rounder overall result.
