Before and after bilateral breast revision with liposuction of the axillary rolls in a 37-year-old woman. She previously had two reconstructive procedures over the last twenty years that involved placement of an implants on both sides to try to correct congenital breast asymmetry and “pectus excavatum” (a caved-in central chest and sternum).

She had a “double bubble” on the right resulting from a submuscular position of the implant that sat lower than her native inframammary fold, known as “bottoming out” of the implant. Flexion of her chest muscles accentuated this deformity, as shown in the photos. Conversion of the right implant to sit on top of the muscle in the subglandular position was the solution!

Her revision involved reconstruction of the pectoralis major muscle’s attachments to the chest wall, capsulectomy and implant exchange on the right, with a left balancing mastopexy (breast lift). She was very happy with her left-sided implant that had a minimal animation change, so we left it alone. Liposuction removed excess fat primarily from the armpit region and bra rolls.

Follow up photos are shown 6 months after surgery, with improved symmetry and balance to her breasts.
