Before and after bilateral breast augmentation revision and mastopexy in a 44-year-old woman.  She felt that her current breast implant size was too big for her body, and she had made symptomatic hypertrophic scars around her areolas that were itchy and painful.

She did not want to go to sleep for surgery and was relieved to learn that an implant exchange and scar revision could be done awake, under local anesthesia.  Her existing scars under the breasts were used to remove her old implants, and she helped to choose her new implant size via intraoperative sizing with a temporary saline sizer.

She chose a new implant around 100 cc smaller than her current implant size.  The new implants were inserted into the same breast implant pocket, and her areola scars were surgically revised using exacting surgical technique and non-reactive monofilament dissolving sutures.  She began applying scar gel one month after surgery and wore adhesive scar strips at night to help them flatten sooner.

Follow up photos are shown 4 months after surgery.  She feels more comfortable with her smaller breasts but still proportional with the rest of her body, and her scars are healing without symptoms.
