Before and after breast revision in a 37 year old woman.  Her old saline implants were contracted and droopy, with unsightly hypertrophic scars around the nipples and areolas. 

Her implants were previously inserted through the bottom of the areola (peri-areolar incision) which can create a puckering deformity from scar contraction, visible in her before images.  She wanted to keep implants but was going for a more youthful look with a better cosmetic outcome. 

Her past surgeon had attempted to lift the breast by only making an incision around the areola (“Benelli” or “donut” mastopexy) – this often results in an areola that is stretched out and even larger than before.  Generally, a vertical incision is necessary to take tension off the circular incision during healing. 

Her saline implants were removed through a new incision in the fold under her breast (inframammary incision).  New silicone gel breast implants were placed in a higher and more youthful position.  Her old areola scars were removed and a proper breast lift created a new lollipop scar. 

Follow up images are shown 6 months after surgery.  Her breast shape is improved, her pink scars are starting to fade and there is no more areola puckering deformity.
