Before and after bilateral breast revision from implant based reconstructions with textured implants under the muscle to bilateral DIEP flaps in a 54 year old woman. She was unhappy with her reconstruction result and was unable to exercise due to constant muscle spasms. Her animation deformity was significant and caused pain.

Both implants were removed, her pectoralis major muscles were returned to her chest wall, and lower abdominal skin and fat was transplanted microsurgically to her chest as DIEP flaps. Her discomfort and muscle spasms were gone as soon as she woke up from her DIEP flap surgery. One year later, she underwent free fat grafting to contour her donor site further and bilateral medical tattoo was performed for nipple and areola reconstruction.

Follow up photos are shown three months after her second stage procedure. The tattoo is still very bright; it will gradually fade to a more normal hue and intensity with time.
