42-year-old woman with left breast cancer who wished to be reconstructed smaller and lifted than her preoperative breast size. Although she was a good candidate for a flap reconstruction, she opted for an immediate implant reconstruction to enable her to return to her job and young family as soon as possible. A breast reduction pattern-mastectomy on the left was used to lift and reduce her reconstructed breast. An immediate nipple and areola reconstruction using a free graft was done during this single procedure to complete the reconstruction. A balancing right breast reduction achieved good symmetry with the reconstructed side, shown at 3 months after surgery.
