Before and after breast revision in a 49 year woman with 20 year old breast implants.  They were both ruptured and under the muscle.  She wanted to keep implants but wanted a more normal appearance to her breasts, and to have a softer outcome. 

She had several sequelae of having implants under the muscle:  a severe animation/flexion deformity when she flexed her pectoralis muscles, the “Snoopy nose” deformity where the breast tissue “falls off” the implant which is pulled abnormally high on the chest by the pec muscle, and capsular contracture. 

Her old inframammary fold scars were used to access the implants.  The old implants were removed and the pocket was cleaned out, her pectoralis major muscles were returned to the chest wall, and new smooth round silicone gel breast implants were placed on top of the muscle. 

Follow up photos are shown three weeks after surgery.  She had drains in place for a week, and has just been given permission to exercise.
