Patient # 387
Before and after bilateral breast revision with liposuction of the axillary rolls in a 56-year-old BRCA gene-positive woman who previously had undergone bilateral nipple-sparing mastectomies to reduce her risk of breast cancer. She had pain, breast asymmetry and a result that she was embarrassed to show anyone. She was looking for a smaller, more natural looking outcome and better symmetry.
Her revision involved bilateral capsulectomy to remove excess scar tissue around the implants, breast implant exchange to a smaller and lower profile device, a lift of her left nipple and areola via a crescentic incision on the top of the left areola, and liposuction of the axillary rolls to remove unwanted and disfiguring armpit fat.
Her stiff and folded breast implants (from capsular contracture) were removed and returned to the manufacturer, as they were covered under the implant company’s 10-year warranty which supports women with Grade IV capsular contracture. New softer smooth round silicone gel breast implants were placed and kept on top of the muscle.
Thankfully, her implants were already in the prepectoral position (on top of the muscle), so her recovery was a little less involved than converting implants from under the muscle to on top – which is our usual breast revision technique. She still required drains for around a week, and took a full month off work to recover.
Follow up photos are shown 3 months after surgery, with greatly improved symmetry, maturing scars and a much more physically comfortable patient.
