63-year-old woman with a history of right breast cancer, mastectomy and tissue expander reconstruction 11 years previously. She never proceeded with an implant exchange due to dissatisfaction with the initial cosmetic outcome. She wished to have conversion of her implant-based reconstruction to a flap reconstruction. Her tissue expander was removed, her pectoralis major muscle was returned to her chest wall and reconstructed, and a DIEP flap reconstructed her right breast. Postoperative images show a flatter tummy, better breast symmetry and a youthful and athletic body contour!
