54-year-old woman with a history of left breast cancer, chemotherapy and radiation. She wished to use her abdominal tissue for breast reconstruction but spare her abdominal muscles, and to have slightly smaller and perkier breasts than her right side. A DIEP flap reconstructed her left breast, while a right breast reduction achieved her goals. Her left nipple and areola was reconstructed using a free nipple graft from the right side and a medical tattoo as a short in-office procedure.
