Before and after bilateral immediate breast implant reconstruction in a 35 year old BRCA gene positive woman.  She had finished having children and disliked the deflation and drooping of her breasts. Her aesthetic goals were to be reconstructed as full as she was before having children and like when she was breast feeding.  

Bilateral nipple sparing mastectomies were performed through a breast lift incision.  Her nipples and areolas were saved as free nipple grafts and reapplied at the end of the surgery in a higher and more youthful position.  

Smooth round adjustable saline breast implants were placed at the time of mastectomies and inflated to their full volume before the patient awoke.  Her implants were placed in the prepectoral position, on top of the chest pectoralis major muscle, as is the standard for all of our patients since 2006.  

She waited three months and returned to the operating room for exchange of her saline filled breast implants to silicone gel.  

Follow up photos are shown 6 months after her mastectomies and three months after breast implant exchange.  Her previously empty breast skin (especially noticeable with her arms raised overhead) is now filled out by her breast reconstructions.  She has returned to body building and loves her new reconstructed breasts!
